Affordable Family Prices with Adult Adjustment:

Children ages 5 and under – $5

Children ages 5 to 18 –Cost is equivalent to the age of the patient (e.g. $10 for a 10-year-old)

Example:A family of 4 (one adult and three children ages 13, 7, 5) $30 + $13 +$7 + $5 = $55
